- Sport Physicals – If your high school student will be participating in a Fall sport, he or she will need to have a physical prior to the first day of the official PIAA season. Sports physicals for the upcoming 20-21 school year will be offered to all Canton Jr and Sr high school students at the high school on 7/15/20 for a fee of $20 per student athlete. Additional details including specific times of arrival for student athletes will be posted shortly. Please complete sections 1-6 of the PIAA Comprehensive Initial Pre-Participation Physical Evaluation Form below prior to arriving on 7/15/20 to prevent delays. Forms are also located inside the main lobby of the high school. Recertification Information If a student has already competed in a sport in the same school year and previously completed the PIAA Physical Form Sections 1-6, then Section 7 of this form must be completed for any subsequent sports seasons (winter/spring).
