- Building Use Rules and Regulations
- Public Building Use Form - Anyone wishing to use any district building or grounds, must fill out a building use form and submit it to the district by fax (570)673-3680 or email email@example.com
- Facility Use Acknowledgement form - Groups or individuals wishing to use any building or grounds must also read the CASD Covid-19 Health and Safety Plan, and/or CASD Athletic / Co-Curricular Health and Safety Plan, agree to abide by it and return this signed form.
- Rules for Gym Floor Use
- CASD employees must use the "Employee form" located in the Business Office forms section of the website. If you are requesting on behalf of an outside organization, you must use the public form. Please fill out and submit to the building prinicipal.
